Frank Strada, the prison chief of Tennessee, is resigning this month. Governor Bill Lee confirmed this decision. Strada faced heavy pressure from Republican leaders and lawmakers to step down. This happened because the execution of Christa Gail Pike completely failed. She is the only woman sentenced to death in Tennessee in the last 200 years. The medical team messed up the lethal injection, leaving her unconscious and on a ventilator in a hospital. This is the second time since May that Tennessee officials failed to complete an execution and brought an inmate right to the edge of death.

Governor Bill Lee has appointed former US Attorney Ed Stanton to investigate what exactly went wrong inside Pike's execution chamber. Stanton is the same person who investigated another cancelled execution back in 2022. Back then, he found that the Tennessee government did not follow its own rules for lethal injections. Frank Strada released a statement saying his resignation is the best move for the people of Tennessee while Stanton's probe is going on. The execution team actually pushed two doses of the lethal drug 'pentobarbital' into Pike's body, but the execution still failed. Pike is currently 50 years old. A court sentenced her to death in 1995 when she was just 18. She and her boyfriend brutally tortured and murdered her classmate Colleen Slemmer out of jealousy. This made her one of the youngest women in America to get the death penalty.

Pike's lawyers claim the execution team messed up the IV lines. The lethal drug leaked into other parts of her body instead of going straight into her veins. The lawyers added that Pike fully cooperated with the execution team and even pointed out the veins on her fingers and hands for the IV. The whole incident has sparked a massive controversy. Both supporters and opponents of the death penalty are protesting strongly. Governor Lee has now put all new executions on hold. Earlier in May, officials had to stop the execution of another inmate, Tony Carruthers, at the last minute because they could not connect the IV line properly. The Governor then ordered a wait of at least one year before trying to execute him again.

Frank Strada took charge as the Tennessee commissioner in January 2023 after leaving the Arizona prison department. He changed the lethal injection rules in late 2024 and decided to use only pentobarbital. Court records show that executions under Strada's watch in Arizona also faced similar technical glitches and complications. When Strada supervised executions in Arizona, the medical teams often failed to find the inmates' veins. They had to insert IV lines into the groin and hands, which delayed the whole process. Human rights lawyers are now criticizing the Tennessee Governor. They say the Governor appointed Strada even after knowing his bad track record with executions.
